Here is how to use the online Application/Renewal Form if you don't have an online account, yet:

• 1. Create an Account
• 2. Enter your email for both the user name and email fields
• 3. Go to your email and follow the one-time link in your email to log in
• 4. After logging in, change your password
• 5. Click on Application/Renewal Form
• 6. Follow instructions on the screen to complete the form
